There is so much interesting extensions not yet available as RPM...

Laptop:  Fedora 38 + rpmfusion + remi (SCL only)
x86_64 builder: Fedora 39 + rpmfusion + remi-test
aarch64 builder: RHEL 9 with EPEL
Hosting Server: CentOS 8 Stream with EPEL, rpmfusion, remi

Try php-pecl-cairo from remi repo (or remi-test for php 5.4).

Which distro ?

RPM are available for Fedora from 11 to 17 and Enterprise Linux 6.

This extension is not compatible with EL-5 (doesn't build), cairo version (1.2.4) is too old (need > 1.4)
